To register your child and guarantee your child a place at reborn, you need to complete and attach the following documents:

 a. Registration form (date of birth, Child's name spelt correctly & Emergency phone number)

  b. Medical form, stamped from the Doctor
  c. Transportation application form (if applicable)   

  d. Photograph disclaimer form
 

Copies of your child's

  a. Civil ID
  b. Birth Vaccination's Certificate
  c. Parents Civil ID's
  d. 6 passport photographs of the student
 e.Copy of Passport and Residency page (NON Kuwaitis)
